The LGHIB has selected UnitedHealthcare® (UHC) to offer a custom UnitedHealthcare® Group Medicare Advantage (PPO) plan for all eligible retirees and their Medicare-eligible dependents. This plan has been exclusively designed for the LGHIB retirees. Only eligible retirees of local government units who have chosen to provide Medicare retiree coverage and their Medicare-eligible dependents can enroll in this plan.
As a UHC Medicare Advantage member, your plan will help give you value for your health care dollar, offering benefits and services beyond what you will find with Original Medicare (Parts A and B). You will have a team committed to understanding your needs, connecting you to the care you need and helping you manage your health. UHC will link you to health and wellness resources and even schedule your wellness visits, including arranging an annual visit.

UnitedHealthcare® House Calls1
A yearly preventive care check-in that helps you stay on track with your health between your regular provider visits. It’s done in the comfort of your home and allows plenty of time to ask questions about your health. A licensed health care practitioner will review your health history and current medications, perform a health screening, refer you to other health services if needed, identify health risks and provide health education.
We send a summary of your HouseCalls visit to you and your provider so your provider stays informed. The visit is available at no cost to you.

Speak to a registered nurse 24/7 about your medical concerns, including:
- Guidance for symptoms and directing you to the appropriate level of care
- Questions about a medication
- Understanding an ongoing health condition or new diagnosis

Renew Active® by UnitedHealthcare3
Renew Active is the gold standard in Medicare fitness programs for body and mind, available at no additional cost. You’ll receive a free gym membership with access to the largest Medicare fitness network of gyms and fitness locations. This includes access to many premium gyms, on-demand digital workout videos and live streaming classes, social activities and access to an online Fitbit® Community for Renew Active and access to an online brain health program from AARP® Staying Sharp® (no Fitbit device is needed).
Virtual Coaching Programs
Rally CoachTM programs will help you start living a healthier, happier life with:
- Real Appeal®, an online weight loss program proven to help you achieve lifelong results, one step at a time (includes a diabetes prevention program for those who qualify)
- Rally Wellness Coaching, which helps you get healthy your way by providing 24/7 access to digital health and wellness courses as well as personalized coaching support with online chat or phone calls
- The Quit for Life® Tobacco Cessation Program, which provides you with the support you may need to quit all types of tobacco use
UnitedHealthcare Healthy at Home
You get benefits up to 30 days after discharge from inpatient and skilled nursing facilities with a $0 copay through UnitedHealthcare Healthy at Home. After you have been discharged, you are eligible to receive 28 home-delivered meals4, 12 one-way rides to medically-related appointments and the pharmacy5, and 6 hours of in-home personal care for tasks such as meal preparation, bathing, medication management and more6.
Don't forget to follow up with your primary care provider (PCP) as soon as possible after a hospital or skilled nursing facility stay.
Personal Emergency Response System (PERS)
With the Personal Emergency Response System (PERS), provided by Lifeline, help is a button push away. The PERS device can quickly connect you to the help you need, 24 hours a day in any situation. It’s a lightweight, discreet button that can be worn on your wrist or as a pendant. It’s also safe to wear in the shower or bath. Depending on the model you choose, it may even automatically detect falls7.
Custom-Programmed Hearing Aids
With UnitedHealthcare Hearing, you’ll get access to hundreds of name-brand and private-labeled hearing aids — available in-person at any of our 7,000+8 UnitedHealthcare Hearing providers nationwide9 or delivered to your doorstep (select products only).
You have a $500 allowance to use towards the cost of hearing aids every 3 years (over the counter not included in allowance). Choose from hearing aid models and styles from the industry’s top brands, featuring advanced technology for superior sound quality, including Bluetooth® streaming, recharging capabilities, hands-free calls with tap control and more.
